Patient Pre-Operative Screening

It’s important to be prepared for you upcoming surgery. To help, we request that you call our surgical staff within two days of receiving this notice. To help us provide a quality experience, please have the following items available when you call 208-690-9228:

• Medications you are taking, including names and dosages.
• List of your allergies to drugs, food or environment.
• Your current health problems and the doctors you see.
• Any previous surgeries you may have had.
• Any special needs or requirements
• Or any other concerns or questions you may have.

Driving Directions

If traveling from outside of Blackfoot, take exit 93 off I-15 and turn east towards Blackfoot on Bergener Boulevard. Follow Bergener Boulevard, which will become a one way street called Judicial Street. Turn left onto Oak Street. Go straight through two intersections and take a slight left on Poplar Street. The main hospital will be on your right.

Patient parking is available in various areas around the hospital. Surgery is on the second floor; we will be happy to guide you there from any entrance.
