Illnesses and injuries that require hospitalization can bring change and stress to patients and their loved ones. Licensed Social Workers at Bingham Memorial Hospital work with patients and families to plan for any supplies or assistance patients may need after their hospital visit.

The Case Management staff is available to assist in arranging Home Health Care, a transfer to an Extended Care Facility, or to a Rehabilitation Center. They will also help patients make arrangements for Outpatient Rehabilitation Services and obtain medical equipment such as a cane, walker, or wheelchair.
